 1. Group Name / Performer Name
 2. Representation Name
 3. Department / Position
 4. Representative Contact Information (Email Address)
 5. Number of participants and Musical instrument organization
 6. Titles of the song, Composer, and Performance Time (up to 10 minutes)
 7. Instruments to bring, Equipment / Whether to use the Grand Piano
 8. Message（convenient times of day for playing, other requests)

※ Due to the covit-19 virus, priority is given to member of U-Tokyo.
※ In case of a large number of applicants, there will be a screening.

※ The final selection results will be announced on early December.
